About the Role
Stern at Home Therapy is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our team full-time in LeRoy, NY. This unique position provides the opportunity to treat patients both within our facility and throughout the surrounding community, allowing you to build meaningful relationships while delivering high-quality, patient-centered care.
Working under the supervision of a licensed Physical Therapist, you'll help patients regain mobility, improve function, and maximize independence across a variety of clinical settings.
Responsibilities:
- Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist.
- Deliver care to patients in our LeRoy facility and throughout the surrounding community.
- Carry out individualized treatment plans designed to improve mobility, strength, balance, endurance, and functional independence.
- Monitor the patient's progress and communicate any changes to the supervising Physical Therapist.
- Accurately document treatments, patient responses, and progress in the electronic medical record.
- Educate patients and caregivers on therapeutic exercises, mobility techniques, safety strategies, and home exercise programs.
- Collaborate with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, physicians, nursing staff, and other healthcare professionals.
- Assist with home and community environment observations, as appropriate, to support patient safety and treatment planning.
- Participate in interdisciplinary care planning and quality improvement initiatives.
- Maintain compliance with all federal, state, company, and professional practice standards.
Qualifications:
- Associate's degree from an accredited Physical Therapist Assistant program.
- Current New York State Physical Therapist Assistant license (required).
- Previous experience in skilled nursing, outpatient, home health, or community-based rehabilitation preferred.
- Comfortable treating patients in both facility and community settings.
- Excellent communication, organization, and patient care skills.
- Strong documentation and electronic medical record (EHR) experience preferred.
- Valid driver's license and reliable transportation required for community visits.
Compensation:
- Competitive salary or pay-per-visit compensation based on experience.
- W-2 or 1099 Employment.
