Physical Therapist (PT) or Physical Therapist Assistant (PTA) – Home Health

Job Summary

Homeside Rehab is seeking a compassionate and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our home health team in Oswego County, NY 13135.

As a valued member of our team, you will provide individualized, evidence-based therapy to patients in their homes while enjoying schedule flexibility and support from our clinical and administrative team.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Provide evidence-based physical therapy interventions to improve mobility, strength, balance, functional independence, and overall quality of life

• Maintain accurate and timely documentation of all patient care in accordance with regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications

• Active New York State Physical Therapy license appropriate to discipline

• Basic computer proficiency for electronic documentation

• Strong clinical assessment and interpersonal communication skills

• Ability to work independently in a home health environment

• Valid driver's license and reliable transportation for home visits

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred

• Job Type: Full-Time, Part-Time, Per Diem

• Location: Oswego County, NY 13135

• Salary: $45–$80 per hour

• Flexible scheduling

• Weekend availability as needed
