    MyCHN Home Health Services - Urgent Hiring for Highly Skilled Physical Therapist Assistant (PTA)

    As a compassionate and dedicated Physical Therapist Assistant (PTA), you will play a vital role in helping patients achieve their rehabilitation goals and improve their quality of life. Working under the direction and supervision of a licensed Physical Therapist, you will provide coordinated care to patients of all age groups.     Duties and Responsibilities:

    • Provide physical therapy treatments and interventions as outlined in the Plan of Care established by the supervising Physical Therapist.
    • Observe, monitor, and document patient responses to treatment and communicate findings to the Physical Therapist.
    • Assist patients in improving mobility, strength, balance, endurance, coordination, and functional independence.
    • Instruct patients and caregivers on prescribed therapeutic exercises, home exercise programs, and safety techniques.
    • Recognize changes in patient condition and promptly report concerns to the supervising Physical Therapist and care team.
    • Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
    • Assist with evaluating and recommending durable medical equipment and adaptive devices as directed by the Physical Therapist.
    • Apply infection control principles to prevent contamination and ensure patient safety.
    • Participate in mandatory continuing education and in-service programs.

    Required Skills/Abilities:

    • Strong therapy skills and knowledge of accepted physical therapy practices.
    • Excellent interpersonal and communication skills.
    • Ability to work effectively under the supervision of a licensed Physical Therapist.
    • Knowledge of durable medical equipment and assistive devices.
    • Strong organizational and documentation skills.
    • Ability to work a flexible schedule and travel locally.
    • Reliable transportation and valid auto liability insurance.

    Required Education and Experience:

    • Graduate of an accredited Physical Therapist Assistant program.
    • Current Texas State License as a Physical Therapist Assistant.
    • Minimum one (1) year of experience in home health, acute care, rehabilitation, or a related healthcare setting preferred.
    • Current CPR certification preferred.

    Physical Requirements:

    • Must be able to lift and/or move up to 50 pounds.
    • Frequent standing, walking, bending, reaching, and hand dexterity required.
    • Ability to assist patients with transfers, mobility activities, and therapeutic exercises.
    • Close vision, color vision, and depth perception abilities needed.

    Work Location & Travel Requirements:

    • Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
    • This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
