Job Summary
Nexus at Westmont is seeking a reliable and compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team on a Per Diem (PRN) basis. This position supports weekday coverage as needed, with additional availability for holiday coverage.
Due to recent staffing changes—one PRN PTA transitioning to another position and another pending work authorization—we are looking to add a flexible team member who can help ensure consistent patient coverage and continuity of care.
The PTA will work under the supervision of a licensed Physical Therapist to provide high-quality rehabilitative services in a subacute rehabilitation and skilled nursing setting, supporting residents in achieving improved mobility, strength, and functional independence.
Responsibilities
- Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist
- Implement individualized treatment plans including therapeutic exercises, gait training, transfer training, and functional mobility activities
- Monitor patient response to treatment and report progress or concerns to the supervising PT
- Assist in maintaining accurate and timely documentation in accordance with facility and regulatory standards
- Support patients with mobility training and safety awareness strategies
- Collaborate with the interdisciplinary team to promote optimal patient outcomes
- Assist with education of patients and caregivers regarding home exercise programs and safe mobility techniques
- Maintain compliance with all facility policies and applicable regulations
Qualifications
- Associate’s degree from an accredited Physical Therapist Assistant program
- Current Illinois Physical Therapist Assistant license in good standing
- Prior experience in skilled nursing or rehabilitation preferred
- Strong communication and teamwork skills
- Ability to work flexible PRN hours, including weekdays and holidays as needed
