Position Summary
Longhorn Village is seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team on a per diem basis. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide skilled therapeutic interventions designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence.

Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans
- Establish functional goals and implement appropriate therapeutic interventions
- Provide skilled physical therapy services including therapeutic exercise, gait training, balance training, transfer training, mobility enhancement, and pain management techniques
- Assess resident progress and modify treatment plans as appropriate
- Supervise and direct Physical Therapist Assistants in accordance with state practice guidelines
- Complete evaluations, progress notes, recertifications, and discharge summaries accurately and timely
- Educate residents, families, and caregivers on treatment plans, exercises, safety techniques, and mobility strategies
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary team members to support resident care plans
- Ensure compliance with all federal, state, and facility regulations and standards
Qualifications
- Current Texas Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Previous experience in skilled nursing, rehabilitation, long-term care, or senior living preferred
- Strong communication and interpersonal skills
- Ability to work independently and within a multidisciplinary team
- CPR certification preferred
