Physical Therapist (DDD Provider)

FamAli LLC partners with Arizona’s Division of Developmental Disabilities (DDD) to deliver high-quality therapy services. We support individuals with developmental disabilities in achieving greater mobility, independence, and participation in daily life.

Key Responsibilities

Conduct DDD-compliant PT evaluations (gross motor, mobility, strength, balance, equipment needs).
Develop and implement Individualized Treatment Plans (ITPs) based on ISP goals.
Provide direct PT services in home, community, or facility settings.
Recommend and train on mobility aids, orthotics, and adaptive equipment.
Participate in ISP meetings and collaborate with DDD Support Coordinators.
Educate caregivers and support staff on therapeutic exercises and safety.
Maintain documentation per AHCCCS and DDD standards.
Monitor progress and adjust treatment plans as needed.
Stay current with Arizona PT licensure and DDD policies.

Qualifications

Doctorate or Master’s in Physical Therapy
Arizona PT license (required)
Experience with developmental disabilities (preferred)
Fingerprint Clearance Card, driver’s license, reliable transportation
Ability to meet DDD credentialing requirements
