Physical Therapist (PT) – Home Health

Location: Northwest Philadelphia & Montgomery County, PA (Coverage Territory)
Job Type: Contract
 

Job Summary

Penn Medicine Home Health is seeking an experienced Physical Therapist (PT) to provide skilled rehabilitative care to patients in their homes throughout Northwest Philadelphia and Montgomery County, PA. The ideal candidate will have recent home health experience, be proficient in EPIC EMR and OASIS documentation, and have experience performing both Start of Care (SOC) and revisit home health visits.

Responsibilities

  • Evaluate and treat patients in their homes to restore mobility, improve strength, balance, endurance, and functional independence.
  • Perform comprehensive Start of Care (SOC) assessments and follow-up (revisit) therapy visits.
  • Complete accurate OASIS assessments and all required clinical documentation.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to ensure continuity of care.
  • Document all patient care accurately and timely in EPIC EMR.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Maintain compliance with CMS regulations and Penn Medicine Home Health policies.

Required Qualifications

  • Active Physical Therapist (PT) license in Pennsylvania.
  • Recent and relevant Home Health Physical Therapy experience is required.
  • Experience with Start of Care (SOC) and revisit home health visits.
  • Proficiency with EPIC EMR.
  • OASIS documentation experience is required.
  • Current BLS Certification (AHA preferred).
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong communication, assessment, and documentation skills.

Preferred Qualifications

  • Experience independently managing a home health caseload.
  • Excellent time management and organizational skills.
  • Strong interdisciplinary collaboration and patient education experience.
