Sunrise Manor Center for Nursing and Rehab is currently recruiting for an IN-HOUSE PRN Physical Therapist for their skilled nursing facility located in Bay Shore, NY. Ideal candidates will have experience working in a skilled nursing environment.

Responsibilities:
* Evaluate, plan and administer treatment to residents and patients with temporary or long term disabilities to relieve pain, restore or improve function, and promote healing
* Assure all treatment is delivered in accordance with an established plan of care
*Provide quality, personalized and evidence-based care and proven intervention to patients with a variety of personalities
*Offer cost-effective treatments that help improve clients’ motion and mobility
* Provide clinical support and instruct patients, families, and caregivers; consult with other health professionals
* Monitor patient response to treatment intervention
* Complete required forms and documentation in accordance with company policy and state/federal regulations
