***$6,000 Signing Bonus***Job Title: Physical TherapistLocation: Canandaigua site - Preschool & ClinicSalary: $58,900 - $72,000 per year The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, internal equity, and location. **This position is eligible for a $6,000 signing bonus! This bonus is earned and payable as follows: 25% ($1,500) in the pay period following the 6-month anniversary of your employment; 25% ($1,500) in the pay period following the one-year anniversary of your employment; and 50% ($3,000) in the pay period following the two-year anniversary of your employment. **Employment: Full Time, School Year Position (37.5 hours per week). Includes 6 week Summer program. Monday through Friday, 8:00am-4:00pm. Evening work may be required as job duties demand.Summary/Objective: CP Rochester/Happiness House exists to support people with physical and developmental disabilities in choosing and accomplishing successive individualized life goals. We do this by providing a wide range of services to people of all ages in the Finger Lakes and surrounding greater Rochester area. Our purpose is to ensure that those we serve safely determine their own pathways in life and live as full members of their communities.Minimum Qualifications: 1. Bachelors degree in Applied Science. Completion of accredited Physical Therapy program. 2. License or eligible for license as a Physical Therapist in New York State. 3. Current New York State registration as a Physical Therapist. 4. Valid New York State drivers license. 5. Experience working with individuals with developmental disabilities preferred. 6. Proficient computer skills with knowledge of Microsoft Office programs. 7. Must have demonstrated commitment to performing job duties and tasks at a high level of performance.Competency Requirements: 1. Client focus. 2. Strong collaboration skills. 3. Must remain in compliance with mandatory trainings per agency policy and per all agencies and governmental entities that are billed and govern the various clinics. 6. Maintains CPR and First Aid Certification.Responsibilities include: 1. Imparts agency philosophy of Equal Opportunity, Independence and Realization of Individual Potential to children, families, program staff and community resources. 2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. 3. Upholds NYS Education Law/Practice Act as relating to the practice of physical therapy. 4. Performs appropriate evaluations to individuals to determine the need for physical therapy services. 5. Ensures valid prescriptions are written for all individuals on caseload. 6. Carries out treatment plans developed after evaluation, in clinic setting for the Article 16 and Article 28 outpatient clinics as well as the Preschool or School Programs, as necessary. 7. Available to consult with other professionals such as teachers, habilitation staff, as well as other members of the interdisciplinary team as appropriate, to facilitate delivery of coordinated quality services to individuals. 8. Works with legal guardians, house staff or parents as integral members of the interdisciplinary team in the clinic, day hab or school setting, as needed. 9. Communicates with physicians, orthopedists and other medical personnel in order to facilitate evaluation and treatment of individuals. 10. Completes all paperwork in accordance with agency, program and department policies/guidelines. 11. Ensures knowledge of paperwork, Electronic Health Record and applicable Medicare and Medicaid regulations in order to provide appropriate documentation. 12. Attends all team, discipline and staff meetings as designated.
