LIFE: Living Independence for the Elderly
Schedule: Monday Friday, 8:00 AM 4:30 PM
Location: Regular home visits in Mercer County & North Burlington County
No on-call duties or weekend work required
About the Role:
client Health's LIFE Program is seeking a Home Health Physical Therapist to help elderly patients maintain their independence. This role involves providing therapy services in participants' homes, creating personalized treatment plans, and collaborating with a dedicated care team.
Key Responsibilities:
Conduct patient evaluations and develop individualized treatment plans.
Provide therapy to improve mobility, function, and overall quality of life.
Educate patients and caregivers on exercises and home-based care.
Work closely with physicians, nurses, and case managers.
Maintain accurate and timely documentation.
Qualifications:
Graduate of an accredited Physical Therapy program.
Licensed Physical Therapist in NJ.
AHA BLS Certification required.
Minimum 2 years of experience working with adult geriatrics.
Experience in home health and senior care is essential.
Strong communication, documentation, and problem-solving skills.
Reliable transportation required.
Preferred Qualifications:
Experience in pelvic floor therapy.
Training in Herman & Wallace Level 1A or equivalent.
Physical Demands & Work Environment:
Frequent standing, walking, and driving.
Occasional lifting (up to 50 lbs.), bending, and reaching.
