Job Summary and Qualifications This is Full Time Day shift position. Hours are 8:00am - 4:30pm with rotating weekends The Physical Therapist (PT) provides high quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient's physical therapy to support patients throughout the continuum of care. Your role will include:Provides evaluations and treatment interventions within the scope of practice established by law and professional code of ethics. Assesses and evaluates therapeutic/rehabilitative/functional status, and participates in the development of the total plan of care. Provides purposeful and skilled therapeutic care at patient's home, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.Observes and reports patient's reaction to treatment and changes in patient's condition. Instructs patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.Ensures accurate, complete, and timely clinical documentation in accordance with service/department guidelines.Communicates pertinent payor needs/requirements to physician, treatment team, patient and patient's family in a timely manner.Prioritizes caseload and, as appropriate, determines daily staffing needs and coordinates assignments/handoffs. Demonstrates efficient and appropriate use of support staff (technicians, PTA's, other clinicians, and office staff). Provides a safe, secure, and infection/hazard-free environment.Serves as an educational resource to patients, families, interdisciplinary team members, payor representatives, physicians, discharge planners, therapists, community members, and other parties as appropriate regarding rehabilitation issues.Maintains current knowledge of and adherence to federal, state, and local regulations. Assumes accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.Maintains a patient-first philosophy and supports the development and implementation of strategies to elevate the patient's experience. Participates as an engaged, active member of the health care team and positively promotes the rest of the team to patients/families/caregivers and other colleagues.Provides general supervision for PT Assistants, as applicable. Communicates clearly and leads in a way that inspires commitment to the vision/mission/values of the organization.
