Physical Therapist

Location: New York, NY 10025
Contract: 13 Weeks
Shift: Day | Monday–Friday
Time: 8:30 AM–4:30 PM
Hours: 40 Hours/Week
Pay Rate: $1,500–$2,000/week
Weekend: Occasional; weekday off provided if worked

Requirements

• Active New York State Physical Therapy License

• Bachelor's degree in Physical Therapy or qualifying program

• Inpatient Rehabilitation experience required

• 0–1 year professional experience may be considered

• Current APTA membership

• Strong clinical assessment and problem-solving skills

• Excellent communication and teamwork skills

• Microsoft Office proficiency

• Strong documentation skills

• English proficiency required

Responsibilities

• Perform physical therapy evaluations and assessments.

• Develop individualized treatment plans and goals.

• Provide evidence-based physical therapy interventions.

• Monitor patient progress and modify treatment plans.

• Coordinate care with physicians, nurses, and rehabilitation professionals.

• Complete evaluations, progress notes, discharge summaries, and other documentation.

• Educate patients, families, and caregivers.

• Recommend DME and prepare medical necessity documentation when required.

• Participate in discharge planning.

• Develop home exercise programs.

• Attend rounds, conferences, meetings, and in-services.

• Provide additional patient coverage as needed.

• Maintain accurate attendance, billing, and department records.

• Follow facility policies, safety standards, and professional practices.
