Job Summary
Folkston Park Care & Rehab Center is seeking a compassionate and motivated Per Diem Physical Therapist to join our in-house rehabilitation team. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to help patients improve mobility, restore function, and achieve their highest level of independence.

Responsibilities
- Perform physical therapy evaluations and assessments
- Develop individualized treatment plans based on patient needs and goals
- Implement evidence-based therapeutic interventions
- Monitor patient progress and modify treatment plans as appropriate
- Educate residents and families regarding therapy programs and home exercises
- Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines
- Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and physicians
- Ensure compliance with all safety and infection control standards
- Participate in care planning meetings as needed
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Georgia Physical Therapist license in good standing
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Skilled nursing facility or long-term care experience preferred
- Experience with Net Health documentation system preferred
