Job Overview

Folkston Park Care & Rehab Center is seeking a compassionate and dedicated Physical Therapist (PT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Physical Therapists to provide exceptional rehabilitation services in our skilled nursing and subacute rehabilitation setting.

As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions that restore mobility, improve functional independence, reduce pain, and maximize each resident's quality of life. You will work alongside an experienced interdisciplinary team committed to delivering outstanding rehabilitation outcomes through compassionate, resident-centered care.

Essential Duties and Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders, clinical findings, and resident goals.

  • Provide skilled physical therapy interventions to improve strength, balance, mobility, gait, endurance, coordination, transfers, and functional independence.

  • Evaluate and treat residents recovering from orthopedic, neurological, cardiopulmonary, and medically complex conditions.

  • Monitor resident progress, reassess functional status, and modify treatment plans as appropriate to maximize clinical outcomes.

  • Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries, in accordance with Medicare, Medicaid, state, federal, and facility guidelines.

  • Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, social workers, and other interdisciplinary team members to coordinate resident care.

  • Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, home safety, and the appropriate use of assistive devices.

  • Participate in interdisciplinary team meetings, care plan conferences, and discharge planning.

  • Recommend and train residents in the use of mobility aids, adaptive equipment, and other assistive devices.

  • Maintain compliance with all professional, regulatory, and facility policies and standards.

  • Foster a positive, resident-centered rehabilitation environment focused on clinical excellence and quality outcomes.

Qualifications

Required

  • Graduate of an accredited Physical Therapy program.

  • Current Georgia Physical Therapist license in good standing.

  • Current CPR certification or willingness to obtain upon hire.

  • Excellent clinical assessment, communication, and documentation skills.

  • Ability to work collaboratively within an interdisciplinary healthcare team.

Preferred

  • Experience in skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.

  • Knowledge of Medicare documentation and reimbursement guidelines.

  • Experience treating geriatric and medically complex patient populations.

  • Familiarity with evidence-based rehabilitation practices, fall prevention, and functional outcome measures.
