Cedarwood Gardens in Franklin, MA has an amazing opportunity for a Full-time and PRN Physical Therapist (PT) to join their In-House Therapy Team.

Cedarwood Gardens serves Norfolk County and is conveniently located near area hospitals. At Franklin, providing unmatched rehabilitation is what we do best. It is our mission to provide our residents with customized rehabilitation programs to ensure optimal recovery and superior care.

Key Responsibilities:

  • Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.
  • Deliver physical therapy services to restore mobility, reduce pain, and improve physical function and independence.
  • Collaborate with an interdisciplinary care team including nurses, occupational therapists, speech therapists, and physicians.
  • Educate patients and families on home exercise programs, safety strategies, and wellness practices.
  • Supervise Physical Therapist Assistants (PTAs) and ensure adherence to treatment plans.
  • Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
  • Participate in care planning, team meetings, and quality improvement initiatives as needed.

 

Qualifications:

  • Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
  • Current State Physical Therapist license in good standing (or eligible).
  • Current CPR certification (or willingness to obtain upon hire).
  • Strong clinical, organizational, and interpersonal skills.
  • Previous experience in a skilled nursing or long-term care setting is preferred but not required.
