Heritage Health Care Center, managed by HHP Lifeshare, is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based physical therapy services to adult and geriatric patients with a wide range of orthopedic, neurological, and medical conditions. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and deliver interventions focused on restoring mobility, reducing pain, improving strength and balance, and maximizing functional independence in alignment with each patient’s goals.
Working within a multidisciplinary care team that may include physicians, nurses, occupational and speech therapists, social workers, and case managers, you will collaborate closely to coordinate care, communicate treatment progress, and contribute to discharge planning and ongoing care recommendations. You will educate patients, families, and caregivers on home exercise programs, safety strategies, and lifestyle modifications that support long-term health and function. Accurate and timely documentation in the electronic medical record will be an essential part of your daily responsibilities, ensuring regulatory compliance and continuity of care.
