Responsibilities
- Evaluate residents referred for physical therapy services.
- Develop individualized treatment plans based on each patient's needs, abilities, and physician recommendations.
- Provide skilled physical therapy treatments to improve mobility, strength, balance, and overall functional independence.
- Establish realistic, measurable goals and monitor patient progress.
- Complete timely documentation, including evaluations, progress notes, recertifications, and discharge summaries.
- Educate residents and family members on treatment plans, exercises, and safe mobility techniques.
- Collaborate with physicians, nursing staff, and other therapy professionals to provide comprehensive, patient-centered care.
- Maintain compliance with HIPAA, facility policies, and all applicable federal and state regulations.
- Perform other duties as assigned by the Director of Rehabilitation.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist (PT) license in the state of Pennsylvania.
- Experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred, but not required.
- Strong communication, documentation, and interpersonal skills.
- Ability to work independently while collaborating effectively with the rehabilitation team.
