Position Responsibilities (Detailed Daily Duties)

The Physical Therapist will deliver high-quality, patient-centered care in the home health setting. Daily responsibilities include, but are not limited to, the following:

Patient Care & Clinical Services

  • Provide skilled physical therapy services to patients in their homes, following physician orders and agency protocols.
  • Conduct comprehensive initial evaluations to assess functional limitations, mobility, strength, balance, pain levels, and safety risks.
  • Develop individualized Plans of Care (POC) that outline measurable goals and appropriate treatment interventions to be followed by the patient, caregiver, and Physical Therapist Assistant (PTA) during subsequent visits.
  • Perform skilled therapeutic interventions including therapeutic exercise, gait training, transfer training, balance training, fall prevention strategies, and patient-specific functional activities.
  • Monitor patient progress and response to treatment, modifying the Plan of Care as indicated based on reassessment findings.

Care Coordination & Communication

  • Maintain consistent communication with the referring physician regarding patient status, progress toward goals, changes in condition, and requests for new or modified orders as needed.
  • Collaborate closely with Physical Therapist Assistants (PTAs), nurses, home health aides, and other interdisciplinary team members to ensure continuity of care.
  • Educate patients and caregivers on exercises, safety techniques, use of assistive devices, and strategies to improve independence and prevent re-injury.

Documentation & Compliance

  • Complete all required clinical documentation accurately and timely, including evaluations, visit notes, progress notes, recertifications, and discharge summaries, in accordance with Medicare and agency guidelines.
  • Ensure documentation supports medical necessity and skilled services provided.
  • Remain compliant with all state, federal, and accrediting body regulations related to home health physical therapy services.

Team Participation & Case Management

  • Actively participate in weekly interdisciplinary team case conferences to discuss patient progress, recertification needs, goal attainment, and discharge planning.
  • Contribute clinical insight to support appropriate discharge timing and transition planning.
  • Participate in quality improvement initiatives and follow best practices in home health care delivery.

Travel & Scheduling

  • Travel daily to patient homes within Harrison County, MS, maintaining an efficient schedule while providing timely and compassionate care.
  • Manage caseload and daily itinerary independently to ensure productivity and patient satisfaction.

Required Qualifications / Experience

  • Current and active Physical Therapy licensure in the State of Mississippi
  • Graduate of an accredited Physical Therapy program
  • Ability to work independently in a home health environment
