Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.Key ActivitiesDevelop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomesDaily documentation is accurate and timelyApplication of standard treatment proceduresTeaching is provided to patients and caregiversQualifications
Qualifications and ExperienceOne year of experience in acute care, rehab or homecare as a Physical TherapistRegistered with the Board of Registration in Massachusetts.Excellent verbal, written and electronic communications skillsComputer proficiency and ability to learn new softwareAbility to prioritize work and adapt to changing situationsTravel required within Mass General Brigham Healthcare at Home’s geographic area
