Physical Therapist - Home Care - Part-Time

Schedule: Part-Time, flexible schedule, would work 2-3 days/week and must complete a total of 10 visits/week.

Location: This position serves home care patients in the Delano, MN area, including Buffalo, St. Michael, Rogers, Maple Plain, Plymouth, Mound, and parts of Minnetonka/Hopkins (western suburbs).

Pay rate: Pay is $125/visit for assessments/evaluations and $85/visit for normal PT visits, mileage reimbursement included!

Position Summary: The Home Care Physical Therapist assesses the patient and develops a care plan tailored to each patient per orders from the Licensed Prescriber under the general supervision of the Director of Nursing. The Physical Therapist addresses patient's deficits in strength, range of motion, flexibility, safety and minimizing pain/discomfort. 

Responsibilities:

• Assess physical, psychological, social and environmental needs of the patient and family.
• Integrate principles of development and disease process including co-morbidities in creating the plan of care.
• Maintain necessary records and use them appropriately for the care of patients.
• Provide patient/family education including patients self care and care provided by family.
• Serve as a case manager for assigned patients and coordinate with all providers involved in patient care including RN, SW, home health aide and other rehab disciplines.
• Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.
• Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.
• Provide skilled clinical care including evaluation and treatment of patients, and the development/implementation of care plans. 
• Anticipate the needs of the population served as well as the unique characteristics of the communities served.
• All other duties as assigned.

Required Skills/Qualifications:

• Must have a current license with the Minnesota Board of Physical Therapy.
• Must have a Bachelors, Masters or Doctorate degree in Physical Therapy.
• At least one year of previous experience as a PT in home care settings.
• Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.
• Proficiency with communication and data processing equipment, clinical apparatus related to home care, and electronic medical record system.
• Must have BLS certification through American Heart Association (AHA) or American Red Cross (ARC).
• Must have reliable transportation for client visits.
