Summary:

The physical therapist provides home care services in community based and facility settings. The position is responsible for providing therapy services to patients enrolled in home health and hospice programs under the direction of the patient’s physician with evaluation and treatment of a patient's condition as it relates to the patient's disabilities, injuries and/or disease process. Quality of care is provided in accordance with state, federal and accreditation body regulations and the mission of Home Care Services.

 

Position Reports To: Clinical Manager and agency leadership

Essential Functions:
  1. Evaluates the patient's rehab potential, functional status (strength, mobility, gait, ROM, endurance, pain), home environment, DME needs, need for community resources and home health/hospice services through skilled observation and by administering age-specific tests.
  2. Interprets evaluation and test results and develops a plan of care with patient specific physical therapy goals to help patients develop, regain or maintain their ability to perform functional mobility and activity in their home environment and community.
  3. Administers skilled treatment interventions as prescribed by the physician that may include but are not limited to utilization of therapeutic exercises/activities, modalities, recommendation of adaptive equipment /orthotics and assistive devices, instructing the patient/family in preventative and rehabilitative home programs to be continued in the therapist’s absence, when indicated.
  4. Reassesses the status of the patient’s functional ability and the effectiveness of the physical therapy plan of care and makes appropriate revisions with the physician’s approval.
  5. Coordinates the delivery of physical therapy services with nursing, social work, aide, therapy, equipment suppliers, community resources and transitions of care which results in effective utilization of resources and timely attainment of achievable rehabilitation and/or palliative care goals.
  6. Participates in weekly interdisciplinary team meetings and agency meetings as assigned.
  7. Records, and reports to physician, the patient’s reaction to treatment, any changes in the patient’s condition, suggested changes in the patient’s orders and implementation of discharge planning procedures.
  8. Maintains documentation of evaluation, reassessment, subsequent visits, coordination of care, discharge or transfer summaries and other clinical record documentation in the electronic medical record.
  9. Instructs and supervises other health team members in physical therapy procedures and techniques performed as an adjunct to the regular physical therapy plan of care.
  10. Required to maintain current BLS certification.
  11. Other duties as assigned.

Qualifications:

Required:

  • BS degree from an CAPTE accredited school
  • Must be licensed to practice physical therapy in Washington State.
  • Communication: Functionally able to listen, read, speak and write using English language. Functional hearing and visual acuity sufficient to detect changes in patient conditions and perform assessments. Effective interpersonal skills. Demonstrates ability to work in collaboration with health team members.
  • Must possess basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.
  • Current, valid driver's license and access to an insured vehicle with the minimum coverage by law.

Desired:

  • One (1) year of recent physical therapy experience
  • Previous homecare, acute care or skilled nursing experience.
Physical/Sensory Demands:

O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday.
