Physical Therapist - Home Health

Job Title: PRN Home Health Physical Therapist- Montgomery County

Position Summary The PRN Home Health Physical Therapist provides skilled in‑home physical therapy services on an as‑needed basis. This role includes evaluating patient needs, developing individualized care plans, and supporting functional independence and recovery.

Responsibilities

• Patient assessments performed in the home setting

• Individualized treatment plans based on physician orders

• Therapeutic interventions to improve mobility, strength, balance, and function

• Patient and caregiver education on exercises, safety, and condition management

• Accurate documentation of progress, plans, and outcomes per regulatory standards

• Coordination with interdisciplinary team members, including nurses and physicians

• Compliance with home health regulations, policies, and safety protocols

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program

• Active state Physical Therapist license

• 1 year of experience preferred

• Home health experience preferred but not required

• Strong clinical assessment and communication skills

• Ability to work independently and manage a flexible schedule

• Reliable transportation and valid driver’s license
