Position OverviewWheeler Staffing Partners is seeking a compassionate
Physical Therapist, Home Health to provide skilled, patient-centered care throughout the Loveland, CO area. This field-based role is responsible for evaluating patients, developing individualized treatment plans, improving mobility and function, and collaborating with an interdisciplinary team to deliver exceptional home health services.
Key Responsibilities
Provide skilled physical therapy services to patients in their homes throughout the assigned coverage area.
Conduct comprehensive patient evaluations to assess mobility, strength, functional limitations, and rehabilitation needs.
Develop and implement individualized treatment plans based on patient assessments and physician orders.
Deliver therapeutic exercises and interventions to improve mobility, balance, strength, and independence.
Monitor patient progress and adjust treatment plans as needed to achieve established goals.
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care.
Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safe rehabilitation practices.
Maintain accurate and timely documentation of patient visits, treatment plans, and clinical progress.
Ensure compliance with applicable healthcare regulations, clinical standards, and organizational policies.
Participate in required on-call coverage and travel to patient homes throughout the assigned service area.
Required Qualifications
Master's degree in Physical Therapy from a Commission on Accreditation in Physical Therapy Education (CAPTE)-accredited program.
Minimum of 9 months of professional experience as a Physical Therapist.
Current, active Physical Therapist license in Colorado.
Valid CPR certification, maintained throughout employment.
Valid driver's license and reliable transportation for travel to patient locations.
Strong clinical assessment, treatment planning, and documentation skills.
Excellent communication, organizational, and patient education abilities.
Ability to work independently in a home-based care environment while collaborating with an interdisciplinary team.
Must successfully complete a background check and drug screening.
Preferred Qualifications
Previous experience in home health, community-based care, or a similar rehabilitation setting.
Experience managing patients with chronic conditions, mobility limitations, and post-acute rehabilitation needs.
Familiarity with home health documentation and care coordination processes.
