Responsibilities

• Deliver evidence-based physical therapy to homebound patients in a timely manner.

• Develop and implement individualized home health treatment plans with measurable goals.

• Document progress notes and patient outcomes in the electronic health record.

• Collaborate with multidisciplinary team to coordinate patient care and discharge planning.

• Monitor patient progress against defined pipelines and adjust care to meet targets.

• Ensure compliance with HIPAA, payer guidelines, and safety policies during home visits.

• Assess home environment and modify treatments to maximize safety and independence.

• Educate patients and caregivers on exercises, safety, and self-management at home.

• Maintain up-to-date clinical skills and stay informed on best practices in home health.

• Participate in continuous professional development and pursue growth opportunities.

• Identify opportunities for process improvements to enhance patient outcomes and efficiency.

Experience/Qualifications

• Licensed Physical Therapist with active state licensure.

• Strong knowledge of HIPAA, payer guidelines, and safety policies.

• Experience delivering home health PT with outcome-based treatment.

• Ability to develop measurable, patient-specific treatment goals.

• Excellent communication with patients, caregivers, and multidisciplinary teams.

• Proficiency in documenting in electronic health records (EHR).

• Competence in home safety assessment and home modification recommendations.

• Commitment to clinical skill maintenance and ongoing professional development.

• Ability to meet targets using care pipelines and route planning.

• On-site only; valid driver's license and reliable transportation.
