Physical therapist - home health

Job Description:

We are seeking a Physical Therapist to support home health services within the assigned coverage area. This role involves providing high‑quality patient care through evaluations, start of care visits, OASIS assessments, and follow‑up/supervisory visits.

Key Responsibilities:

• Perform evaluations, start of cares, OASIS visits, and return/supervisor visits
• Deliver patient‑centered therapy services in a home health setting
• Meet productivity expectations (30 points)
• Travel within the assigned coverage areas

Schedule & Hours:

• Shift: 8:00 AM – 4:30 PM
• Weekly Hour Guarantee: 32 hours guaranteed; up to 40 hours
• Weekend/On‑Call: Varies (approximately every 8 weekends)

Coverage Area: Somerset County and surrounding regions

Compensation & Benefits:

• Competitive hourly pay
• Mileage reimbursed between patient visits (from first to last patient)

Qualifications:

• Physical Therapist (PT) degree required
• Active professional PT certification
• Associate’s Degree (minimum education requirement)
• Occupational health screening required
