Description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation:

Local Pay: $55.00 - $70.00/hour (W-2)

Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend)*

Location: Needham Heights, MA

Weekly Hour Guarantee: 32

On Call/Weekend Requirement: Yes

Coverage Areas: Chestnut Hill, Newton, Roxbury, Jamaica Plain, West Roxbury, Hyde Park, Mattapan

Shift schedule (days, start & end times): Monday - Friday 8-4:30

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)?Y

Key Responsibilities:

  • Perform comprehensive home health physical therapy evaluations, develop individualized plans of care, provide evidence-based therapeutic interventions, complete CMS-compliant documentation, and monitor patient progress while coordinating with physicians and the interdisciplinary care team across the assigned coverage area.
  • Restore and maximize patients' functional independence through pain management, mobility training, patient and caregiver education, timely progress documentation, and communication of changes in condition, ensuring compliance with state regulations, agency policies, and home health best practices.

Must have qualifications/experience:

Current Physical Therapy licensure in state of Massachusetts

Current CPR certification

Must have a current driver's license and vehicle insurance, and access to a dependable vehicle

Ability to work autonomously.

Fast learner and able to onboard quickly.

Strong patient skills in home care with emphasis on post-acute hospitalization and assisted living populations.

Type of certification required: PT license.

Dress code/scrub color: business casual (no jeans)/ no scrub color
