    Job Description

    UPMC Home Health is hiring a Physical Therapist for the Penn Hills and surrounding area.

    You will work Monday - Friday, daylight hours 8:00 am-4:30 pm.

    Home Health allows you flexibility, independence, and one-on-one care with patients in the home setting. Provide age-sensitive professional flexibility, Independence, and one-on-one with patients in the home setting. physical therapy services in the home and community setting, including assessment, intervention, treatment, and instruction to individuals of all ages to maximize functional abilities and ensure optimal care.

    Responsibilities:

    • • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. • Continually develops professional skills by assuming responsibility for ongoing continuing education.
    • • Evaluates, plans, and treats patients in an effective Physical Therapy Program including the use of modalities and assistive devices within the Standards of Physical Therapy Practice.
    • • Identifies develops and participates in process improvement opportunities within the Agency's Home Health or Early Intervention programs that will enhance the quality of services we provide.
    • • Assigns and supervises the Certified Physical Therapy Assistant, if applicable.
    • • Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.
    • • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC towards patients, visitors, staff, peers, physicians, and other departments within the organization.
    • • Maintains and completes all required documentation, orders, Point of Care or written, promptly and accurately within Agency and regulatory standards. Data syncs (transmits information the same day, if applicable.
    • • Advises and consults with the family, inter-intradisciplinary team members, physicians, and other professionals, as appropriate, to ensure optimal patient care.
    • • Provide physical therapy services and participate in the development of a physical therapy plan of treatment, including modalities, frequency, and treatment in collaboration with the referring physician for patients from birth to geriatrics to assist the patient in obtaining maximum functional independence within the standards of physical therapy

    Graduate of a Physical Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association; or is eligible for the National Registration Examination of the American Physical Therapy Association

    Or Two (2 years of appropriate experience as a physical therapist and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking initial qualification as a physical therapist after December 31, 1977, required. One (1 year of Physical Therapy experience to practice without supervision or less than one (1 year if practicing under supervision, i.e., ALF/PCH required. • Prefer Home Health experience.

    Minimum one (1 year clinical experience to practice without supervision or less than one (1 year if practicing under supervision, i.e.,ALF/PCH.May in the course of their job duties be in rooms where medications are stored. No contact with medications is allowed.
