• Pay Rate: $75.00 / Hour (USD)
• Shift / Schedule: Day (5x8 Hours), 8:00 16:30
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day.

Job Summary

This is a Home Health Physical Therapist opportunity in Seaside, OR and surrounding areas. The facility is a Home Health setting operating under Providence Portland Home Health North Coast. The system does a good job grouping clients to minimize drive and windshield time, with an occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day.

Key Responsibilities

• May visit up to 6 patients per day; 30 productivity points standard (1:1 ratio).
• Complete Oasis assessments as required.
• Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app) for charting.
• Provide skilled home health physical therapy services to patients across the assigned service area.

Required Skills

• 1 year of Skilled Home Health Experience required.
• Proficiency with Oasis documentation (required).
• Proficiency with EPIC Remote Client for Home Health and Rover (phone app).
• Must have own transportation.

Education Requirements

• PT License required.
• BCLS (Basic Cardiac Life Support) certification required.

Required Experience

• Minimum 1 year of Skilled Home Health Experience required.

• Attire / Scrub Color: Business casual, no jeans; scrubs allowed but not required (no specific color).
• Parking Cost: Free.
• Mileage Reimbursement: IRS Standard.
• Traveler is expected to have their own transportation.

• Shift cannot accommodate 4x10s or 3x12s.
