What You’ll Do:

• Deliver individualized physical therapy services to home health patients in the community
• Assess patient needs and develop tailored treatment plans
• Document patient progress using Meditech Home Care EMR system
• Educate patients and caregivers on therapy goals and procedures
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Manage patient caseload and documentation efficiently during scheduled hours
• Travel within approximately 50 miles of the service area, utilizing local resources

What You Need:
Required:

• Valid Louisiana Physical Therapy license
• BLS certification
• Minimum 1 year of acute or outpatient care experience
• Strong communication and patient education skills

Preferred:

• Experience with home health settings
• Knowledge of wound care, IV therapy, and patient mobility management
