Provide skilled physical therapy services to home health patients across the Omaha metro areaDevelop individualized treatment plans focused on mobility, strength, safety, and independencePartner closely with nurses, providers, caregivers, and interdisciplinary teams to support patient goalsComplete timely documentation and maintain compliance with all regulatory requirementsEducate patients and families on exercises, safety, and continued care plansBuild trusted relationships with patients while delivering compassionate, high-quality care

What We're Looking For

Licensed Physical Therapist in the state of Nebraska (or eligible for licensure)Previous home health experience strongly preferredMinimum of 1+ years of PT experience requiredStrong clinical judgment, communication, and organizational skillsAbility to work independently while staying connected with the care teamReliable transportation and valid driver's license
