Overview

We are seeking an experienced Physical Therapist (PT) to provide high‑quality home health services across the Marion and Hamilton county areas. This role focuses on delivering patient‑centered care in a home setting, ensuring continuity of treatment, and supporting overall patient recovery and mobility.

Key Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), routine visits, and Discharges for home health patients.

• Deliver comprehensive physical therapy assessments, treatment plans, and ongoing evaluations.

• Provide clinical oversight to Physical Therapy Assistants (PTAs).

• Maintain accurate and timely documentation using EMAR systems, including HCHB (HomeCare HomeBase).

• Meet productivity expectations of 30 points per week.

• Travel between patient homes; mileage reimbursed from first to last visit.

• Collaborate with interdisciplinary care teams to support patient outcomes.

Required Qualifications

• Bachelor’s degree in Physical Therapy (minimum requirement).

• Active PT license in the state of Indiana.

• Home health experience required.

• Proficiency with EMAR systems and HCHB.

• Ability to work independently in a field‑based environment.
