    Physical Therapist (PT)

    $8,000 SignOn Bonus (Full Time position)

    FullTime | PartTime | PRN Opportunities Available

    What You’ll Do as Our Physical Therapist
    • Evaluate and treat residents with cognitive and/or ADL physical functioning disorders

    • Develop and implement individualized plans of care using evidencebased practice

    • Provide skilled, medically necessary therapy to maximize recovery and functional independence

    • Administer rehabilitation interventions including therapeutic exercises, activities, ADL retraining, modalities, and assistive devices

    • Monitor resident progress and adjust treatment plans accordingly

    • Evaluate treatment effectiveness and recommend modifications or referrals as needed

    • Educate residents, families, and caregivers regarding treatment plans and outcomes

    • Communicate effectively with the interdisciplinary team regarding progress and barriers

    • Complete accurate, timely clinical documentation and treatment minutes

    • Provide thorough discharge summaries per regulatory and clinical standards

    • Participate in care conferences, rehab meetings, and required trainings

    • Supervise and support PTAs, rehab aides, and students as assigned

    • Maintain compliance with state practice acts, professional ethics, and facility policies

    This position reports directly to the Director of Rehabilitation Services.

    What We’re Looking for in YOU
    • Bachelor’s degree or higher in Physical Therapy or related field

    • Active state Physical Therapist license

    • 2+ years of experience in a skilled nursing facility preferred

    • Strong clinical reasoning and documentation skills

    • Passion for working with seniors and improving functional outcomes

    • Ability to work collaboratively in a multidisciplinary team
