We are seeking a licensed Physical Therapist to provide home‑based rehabilitation services across Mifflin and Juniata Counties. This role is ideal for a clinician who values autonomy, patient‑centered care, and the opportunity to support individuals in their home environment.
Mileage reimbursement is provided for travel between patient visits.
Perform physical therapy evaluations and deliver individualized treatment plans in patient homes.
Document all care activities accurately and in compliance with regulatory and organizational standards.
Collaborate with interdisciplinary care teams to support patient progress and continuity of care.
Communicate effectively with patients, caregivers, and internal teams.
Maintain professional standards and adhere to all clinical and safety protocols.
Bachelor’s degree in Physical Therapy or higher.
Active Physical Therapist license in the state of Pennsylvania.
Ability to meet all occupational health screening requirements.
Valid driver’s license and reliable transportation.
Home Health experience
Familiarity with HCHB (Homecare Homebase)
Experience with OASIS documentation

On‑Call/Weekend Requirements: None
Certification Required: Active PT License
