Description: Travel Physical Therapist – Home Health / Hospice
Position Overview
We are seeking an experienced Travel Physical Therapist to support our Home Health and Hospice program. This role focuses on delivering high-quality, patient-centered care in the home setting while ensuring compliance with state regulations, Medicare guidelines, and company standards.
Position Details
Weekly Hour Guarantee: 36 hours
Productivity Expectation: 30 points per week
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Weekend Requirement: Every 3rd Saturday
Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient
Dress Code: Navy Blue scrubs
Primary Coverage Areas: Wabash, Huntington, Grant Counties
Secondary Coverage Areas (as needed): Blackford, Delaware, Madison Counties
Key Responsibilities
Provide high-quality therapeutic interventions to promote optimal patient outcomes
Supervise Physical Therapy Assistants (PTAs) as applicable, following company policy and state regulations
Complete all clinical documentation within 24 hours of each visit
Participate in branch meetings and interdisciplinary case conferences as directed by leadership
Maintain schedule flexibility to meet patient care needs
Participate in weekend rotation and be available for observed holidays as required to support patient care demands
Required Qualifications
Current, unencumbered Physical Therapy license in the State of Indiana
Recent Home Health or Home Care experience
Proficiency with HomeCare HomeBase (HCHB)
Strong understanding of Medicare guidelines related to home health services
Valid CPR certification
Valid driver’s license, active vehicle insurance, and access to a reliable vehicle
