Physical Therapist

Job Description

We are hiring a Physical Therapist for Home Health/Hospice services in Medford, Oregon. This is an on‑site contract opportunity with a strong potential for extension based on business needs and performance.

Job Duration: (3+ months, with possibilities of extension)

Shift Schedule: Monday – Friday | 8:00 AM – 4:30 PM
Coverage Area: Jackson County

Key Responsibilities

• Perform Physical Therapy visits including:
• • Start of Care (SOC)
  • Physical Therapy evaluations
  • Recertifications
  • Resumption of Care (ROC)
  • Supervisory visits
• Meet productivity expectations of 30 points per week
• Deliver high‑quality patient care in home health and hospice settings

Required Qualifications

• Doctorate in Physical Therapy
• Active professional Physical Therapy certification
• Ability to complete occupational health screenings as required

Preferred Skills (Nice to Have)

• Prior Home Health experience
• Experience with Home Care Homebase EMR
