Physical Therapist – Home Health (Contract)

Location: Mount Vernon, IL (Fully On‑Site) 

Type: Contract 

Weekly Hour Guarantee: 32 hours 

Work Environment: Home Health (On‑Site Patient Visits)

Overview

This role provides in‑home physical therapy services across multiple counties. The ideal candidate is an independent clinician comfortable performing evaluations and delivering high‑quality care in a home‑based setting.

Key Responsibilities

• Conduct comprehensive in‑home physical therapy evaluations

• Develop and implement individualized treatment plans

• Document clinical findings and progress notes accurately

• Collaborate with interdisciplinary care teams as needed

• Travel between patient homes within assigned coverage areas

• Maintain professional standards and adhere to all regulatory requirements

Coverage Areas

Clinton, Marion, Washington, Jefferson, and Hamilton counties

Schedule & Expectations

• No on‑call or weekend requirements

• Standard daytime schedule (specific hours may vary)

• Mileage reimbursed between first and last patient visit

Required Qualifications

• Bachelor’s degree in Physical Therapy (minimum)

• Active Illinois Physical Therapy License

• Ability to pass required occupational health screenings

• Professional certification as required for PT practice

Preferred Experience

• Home Health experience

• OASIS experience

• HCHB (Homecare Homebase) experience
