As aPhysical Therapist, you will be an integral part of our interdisciplinary team, playing a vital role in helping our residents achieve their highest level of physical function and independence.

About the Role:

As a Physical Therapist at Mountainside Nursing Home, you will be responsible for:

• Evaluating and treating residents with a variety of physical impairments, including those with musculoskeletal, neurological, and cardiovascular conditions

• Designing and implementing individualized treatment plans to promote resident independence, function, and mobility

• Conducting daily evaluations and treatments, as well as providing education and training to residents, family members, and caregivers

• Collaborating with other healthcare professionals, including physicians, occupational therapists, and speech therapists, to provide comprehensive care

• Maintaining accurate and timely documentation of treatment plans, progress notes, and resident data

Additional Responsibilities:

• Participate in care plan meetings and interdisciplinary team conferences

• Develop and maintain effective working relationships with residents, families, and staff members

• Stay current with best practices and standards of physical therapy practice

• Participate in quality improvement initiatives and peer review processes

Requirements:

• Graduate degree in Physical Therapy from an accredited program

• Minimum 1 years of experience in physical therapy, with a strong desire to work in a long-term care setting

• Current licensure in the state of [State] as a Physical Therapist

• Ability to lift, transfer, and support residents with varying weights and abilities
