Physical Therapist

Pay Rate: $40 per hour to $51 per hour
Shift: 8AM-4PM

Responsibilities:

• Perform initial examinations, establish appropriate goals, plan and implement treatment interventions, perform timely re-examinations and modify goals and treatment interventions accordingly.
• Coordinate with other disciplines when indicated.
• Assume responsibility for all documentation requirements in a timely manner, including initial examinations, progress notes, monthly summaries, and discharge summaries.
• Provide training to families or care partners and medical personnel as indicated.
• Recommend appropriate durable medical equipment in coordination with appropriate team members and write letters of medical necessity when indicated.
• Participate in discharge planning, make recommendations for continued physical therapy, and provide home exercise programs when appropriate.
• Attend all appropriate conferences, rounds, in-services, and meetings.
• Be assigned additional patients in the event that coverage is necessary as dictated by staffing needs.
• Attend clinics as assigned.
• Maintain therapy attendance records, patient charges, and statistics.
• Participate in the supervision of Rehab Patient Care Associates, volunteers, and students.
• Follow all company policies and procedures.

Requirements:

• 0 - 1 year of experience
• Current New York State Physical Therapy license
• Must be a current member of the APTA
