Role Description This full-time Physical Therapist role is a hybrid position based in Philadelphia, PA, with a mix of in-person visits in the community and home settings, along with some work-from-home for documentation and care coordination. The Physical Therapist will evaluate patients with complex neurological and rehabilitation needs, develop individualized treatment plans, and deliver evidence-based interventions focused on functional mobility, strength, balance, and safety. Day-to-day responsibilities include providing in-home therapy sessions, collaborating with an interdisciplinary team, educating patients and caregivers on home exercise programs and adaptive strategies, and monitoring progress toward functional goals. The role also involves timely documentation, participation in team meetings, and communication with physicians and other healthcare partners to ensure integrated patient care. The Physical Therapist will support patients in transitioning across care settings, promoting independence and optimal outcomes in their home and community environments.

Qualifications

• Demonstrated clinical expertise in Physical Therapy and Rehabilitation, particularly with neurologic or high-acuity populations.

• Strong foundation in Medicine related to neurology, orthopedics, and general health conditions that impact functional mobility.
• \n
• Proven skills in Patient Care, including assessment, treatment planning, patient and caregiver education, and goal-oriented therapy.
• \n
• Experience providing Home Care or community-based services, with the ability to work safely and effectively in non-institutional settings.
• \n
• Current licensure as a Physical Therapist in Pennsylvania; Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.

• Strong communication, collaboration, and documentation skills, with the ability to work within an interdisciplinary team.
• \n
• Comfort with using electronic health records and basic digital tools for telehealth, documentation, and care coordination.
• \n
• Valid driver’s license, reliable transportation for community and home visits, and ability to meet the physical demands of patient care.
