Physical Therapist / Physical Therapist Assistant (PT/PTA

We are seeking either a Physical Therapist (PT or Physical Therapist Assistant (PTA to join our rehabilitation team serving patients in both outpatient and inpatient settings. Schedule
Typical Four 10-Hour Daylight Shifts
- Monday & Wednesday: Smethport
- Tuesday & Thursday: Shinglehouse
- Weekend & Holiday Rotation: Inpatient coverage
What You'll Do
- Evaluate, treat, and support patients in achieving their mobility and rehabilitation goals (within scope of practice.
- Develop or implement individualized treatment plans designed to improve function, reduce pain, and restore independence.
- Monitor patient progress and communicate updates with the rehabilitation team.
- Educate patients and families on treatment plans, recovery, and home exercise programs.
- Collaborate with physicians and interdisciplinary team members to ensure high-quality patient care.
- Complete accurate and timely documentation.
- Participate in staff meetings, professional development, and quality improvement initiatives.

Physical Therapist (PT
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the practicing state.
- Salary Range: $35.22 - $52.18/hour
Physical Therapist Assistant (PTA
- Associate degree from an accredited Physical Therapist Assistant program.
- Current Physical Therapist Assistant license (or eligibility in the practicing state.
- Salary Range: $22.56 - $32.13/hour
Licensure, Certifications & Clearances (PT & PTA
- BLS or CPR certification required within 30 days of hire
- Act 33 Clearance
- Act 34 Clearance
- Act 73 FBI Clearance
