Responsibilities:
Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
Designs and implements individualized care plans based on patient assessments.
Reviews patients progress and updates documentation and prescriptions as needed.
Leads and directs support staff to help achieve functional goals.
Promotes quality of care in accordance with Elara Carings mission and philosophy.
Conducts therapy practice within defined standards of care.
Participates in community education and outreach.
Qualifications:
Graduate of an American Physical Therapy Association (APTA) approved Physical Therapy program.
Current, unrestricted Physical Therapist (PT) license valid in the state of work.
Minimum one (1) year of experience in a healthcare facility.
Able and willing to travel within branch/office coverage area (up to 50%).
Valid drivers license, reliable vehicle, and current auto insurance.
Ability to frequently lift, push, pull, and support up to 50 pounds.
