Description:
Full-Time - 40 hours weekly; Days - 8 hour shift; New Hampton Medical Center
This individual works collaboratively within a multidisciplinary team to develop
an individualized physical therapy treatment plan for client's home/residence.
ESSENTIAL FUNCTIONS
- Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting.
- Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.
- Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
- Communicates with physician any requests to revise the plan of care.
- Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
POSITION QUALIFICATIONS
- Graduate of a university approved by American Physical Therapy Association
- Licensed in the state of Iowa to practice physical therapy
- Minimum of 1-year experience preferred
- Community or home health experience desirable
