Physical Therapist (PT) – Outpatient

Location: Alpena, MI 49707
Schedule: Full-Time | Days
Shift: Monday – Friday, 9:00 AM – 5:30 PM
Weekend Rotation: 1 per month
On-Call: Discussed during interview
Pay Rate: Starting at $37.00+/hour (based on experience)
Sign-On Bonus: $20,000 (2-year commitment required)

Physical Therapist Position Overview

We are seeking a skilled and motivated Physical Therapist to join a collaborative healthcare team in a primarily outpatient setting, with occasional support in acute care. This role focuses on evaluating, treating, and improving patient mobility and function through individualized care plans.

Physical Therapist Patient Population

• Primarily Outpatient
• Occasional Acute Care coverage

Physical Therapist Key Responsibilities

• Evaluate patient function using formal and informal assessment methods
• Develop individualized, goal-oriented treatment plans
• Implement therapeutic interventions to restore mobility and function
• Monitor patient progress and adjust care plans as needed
• Educate patients, families, and caregivers on treatment and home programs
• Coordinate care with physicians and interdisciplinary teams
• Maintain accurate and timely documentation for compliance and billing
• Participate in care planning meetings and team conferences

Additional Responsibilities

• Supervise Physical Therapist Assistants, aides, and students
• Recommend and assist with adaptive/supportive equipment
• Support program development and process improvement initiatives
• Promote physical therapy services within the community
• Function as case manager for assigned patients
• Ensure compliance with regulatory and organizational standards

Physical Therapist What We’re Looking For

• Licensed Physical Therapist (or eligible) in Michigan
• BLS certification (required within 1 month of hire)
• Bachelor’s degree (minimum requirement)
• Strong clinical evaluation and patient care skills
• Excellent communication and organizational abilities
• Ability to work independently and within a team environment
