Job Description


A Home Health and Hospice Physical Therapist (PT) – PRN is responsible for providing skilled physical therapy services to patients in their homes. The Physical Therapist evaluates patient needs, develops individualized treatment plans, improves mobility and strength, reduces pain, prevents disability, and promotes functional independence while coordinating care with physicians, nurses, and interdisciplinary healthcare teams.


The PRN Physical Therapist works on an as-needed basis and must be flexible with scheduling and patient visits.


Essential Job Duties & Responsibilities


  •  Perform comprehensive physical therapy evaluations and assessments for home health and hospice patients. 
  •  Develop individualized treatment plans based on physician orders and patient needs. 
  •  Provide therapeutic exercises, gait training, balance training, transfer training, and mobility assistance. 
  •  Assess and recommend assistive devices and durable medical equipment as needed. 
  •  Educate patients, families, and caregivers regarding treatment plans, safety precautions, home exercise programs, and fall prevention. 
  •  Monitor patient progress and modify treatment plans accordingly. 
  •  Complete accurate and timely documentation in accordance with agency policies, Medicare guidelines, and state/federal regulations. 
  •  Coordinate patient care with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals. 
  •  Participate in interdisciplinary team meetings and care planning. 
  •  Maintain patient confidentiality and comply with HIPAA regulations. 
  •  Follow infection control, safety, and agency protocols. 
  •  Provide compassionate care while supporting patient dignity and quality of life. 
  •  Travel to patient homes within assigned service areas. 
Education Requirements


  •  Graduate of an accredited Physical Therapy program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE). 
  •  Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) degree required. 
  •  Current and unrestricted Physical Therapist (PT) license in the State of Texas. 
  •  Current CPR/BLS certification required. 
Experience Requirements


  •  Minimum of one (1) year experience as a licensed Physical Therapist preferred. 
  •  Home Health and/or Hospice experience strongly preferred. 
  •  Experience with Medicare documentation and OASIS preferred. 
  •  Knowledge of rehabilitation techniques, safety practices, and patient care standards. 
  •  Experience working with geriatric and medically complex patients preferred.

  •  Excellent clinical assessment and treatment planning skills. 
  •  Strong communication and interpersonal skills. 
  •  Ability to work independently and manage time effectively. 
  •  Compassionate and patient-centered approach to care. 
  •  Knowledge of home health regulations and hospice philosophy. 
  •  Ability to maintain accurate documentation and electronic medical records (EMR). 
  •  Reliable transportation, valid Texas driver’s license, and current auto insurance required. 
Work Schedule


  •  PRN / As Needed 
  •  Flexible scheduling based on patient census and agency needs. 
Physical Requirements


  •  Ability to stand, walk, bend, lift, and assist patients during therapy sessions. 
  •  Ability to lift up to 50 pounds with or without assistance. 
  •  Ability to travel between patient homes.
