Responsibilities<\/span>
<\/h2>- Complete comprehensive patient evaluations in the home setting<\/span>
<\/li> - Develop individualized plans of care based on each patient’s condition, needs, and goals<\/span>
<\/li> - Provide skilled physical or occupational therapy services within the therapist’s licensed scope of practice<\/span>
<\/li> - Monitor patient progress and modify treatment plans when clinically appropriate<\/span>
<\/li> - Complete timely and accurate visit documentation<\/span>
<\/li> - Educate patients, family members, and caregivers regarding treatment plans, safety, mobility, home exercises, and independence<\/span>
<\/li> - Collaborate with physicians, nurses, therapy professionals, and other interdisciplinary team members<\/span>
<\/li> - Communicate changes in the patient’s condition to the appropriate clinical team members<\/span>
<\/li> - Ensure services and documentation comply with agency policies and applicable state and federal regulations<\/span>

Requirements<\/h3>
Physical Therapist Requirements<\/span>
<\/h2>- Active and unrestricted Florida Physical Therapist license<\/span>
<\/li> - Minimum of six months of physical therapy experience<\/span>
<\/li> - Home health experience preferred<\/span>
<\/li> - Strong patient assessment, treatment planning, documentation, and communication skills<\/span>
<\/li><\/ul>Occupational Therapist Requirements<\/span>
<\/h2>- Active and unrestricted Florida Occupational Therapist license<\/span>
<\/li> - Minimum of six months of occupational therapy experience<\/span>
<\/li> - Home health experience preferred<\/span>
<\/li> - Strong patient assessment, treatment planning, documentation, and communication skills<\/span>
<\/li><\/ul><\/span>

- Valid driver’s license<\/span>
<\/li> - Reliable transportation and current automobile insurance<\/span>
<\/li> - Current CPR or Basic Life Support certification preferred<\/span>
<\/li> - Ability to successfully complete an AHCA Level 2 background screening<\/span>
<\/li> - Ability to travel to patients’ homes within the assigned service area<\/span>
<\/li> - Excellent organizational and time -management skills<\/span>
<\/li> - Ability to work independently while collaborating with an interdisciplinary care team<\/span>
