Heritage Home Health Services is currently looking for a Physical Therapist to join our team! The area of coverage will be determined upon location of candidate. This is a pay per visit position.

The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.

Qualifications:

• Ability to perform therapy evaluations, routine visits, and OASIS documentation
• Work with other disciplines to give the best care needed for each patient.
• Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
• When appropriate teach and counsel family members about care and processes for disease/injuries.
• Attend orientation, meetings, and CEUs
