At Clinton Manor Living Center, their mission is to passionately support each person in living a meaningful and satisfying life!Clinton Manor provides long-term care with skilled nursing that accommodates seniors or individuals with high medical needs. A team of highly-skilled RN's, LPN's and Certified Nurse Assistants support residents recuperating from surgery, stroke or illness. Clinton Manor also specializes in assisting people with intellectual and developmental disabilities. Direct Support Professionals (also known as DSPs) provide hands-on, person-centered, direct care for individuals with developmental disabilities whom reside with us.We have now extended out therapy department and looking to find quality candidates to fill supporting roles within this department.The Physical Therapist evaluates and treats residents with physical impairments, injuries, or disabilities in a skilled nursing facility setting, using evidence-based interventions to improve mobility, reduce pain, and maximize functional independence. The PT works as part of the interdisciplinary care team to develop, implement, and modify individualized treatment plans.This is a PRN position and will help out as neededClinton Manor's full benefits listing: https://clintonmanorlivingcenter.com/benefits-list/ Essential Duties & ResponsibilitiesConduct comprehensive evaluations of residents' physical abilities, medical history, and functional goals using standardized tests, measures, and clinical observationDevelop and implement individualized treatment plans utilizing therapeutic exercise, manual therapy, gait and balance training, and other evidence-based interventionsEstablish measurable short- and long-term goals in collaboration with residents, families, and the interdisciplinary teamMonitor and reassess resident progress, modifying treatment plans as clinically indicatedEducate residents and families on home exercise programs, assistive devices, fall prevention, and self-management strategiesCollaborate with physicians, nursing staff, and other therapy disciplines to coordinate care and optimize outcomesMaintain timely, accurate, and compliant documentation of evaluations, treatment sessions, and progress notes to support Medicare Part Band other payer requirementsParticipate in interdisciplinary team meetings, care conferences, and quality improvement initiativesDelegate and oversee care provided by Physical Therapist Assistants in accordance with the state practice actMaintain clinical competency through continuing education and stay current on evidence-based practiceEducation, Certification & licensure RequirementsDoctor of Physical Therapy (DPT) degree from a CAPTE-accredited program (bachelor's or master's degree accepted for therapists licensed before the DPT became the entry-level standard)Successful completion of the National Physical Therapy Examination (NPTE)Current, unrestricted state license (or license eligibility) as a Physical Therapist; multistate practice privileges through the PT Compact are a plus in participating statesCurrent CPR/BLS certification preferredExperience & Preferred SkillsExperience in a skilled nursing facility, long-term care, or geriatric setting preferredWorking knowledge of Medicare Part A/Part B, PDPM, and third-party payer documentation requirementsProficiency with electronic medical record (EMR) systemsStrong clinical reasoning, communication, and interpersonal skillsAbility to manage a caseload independently and as part of an interdisciplinary teamEOE
