Physical Therapist (PT) – Outpatient
Location: Cheraw, South Carolina
Employment Type: Full-Time
The Physical Therapist evaluates patients, develops individualized treatment plans, implements evidence-based rehabilitation interventions, monitors patient progress, and provides education to patients, families, and caregivers. The Physical Therapist collaborates with physicians and interdisciplinary rehabilitation professionals to optimize patient outcomes while ensuring compliance with regulatory and professional standards.
Key Responsibilities
Perform comprehensive physical therapy evaluations.
Develop individualized treatment plans based on patient needs and goals.
Deliver evidence-based physical therapy interventions.
Improve patient mobility, strength, balance, and functional independence.
Monitor patient progress and modify treatment plans as needed.
Educate patients, families, and caregivers on home exercise and follow-up programs.
Collaborate with physicians, nursing staff, and rehabilitation team members.
Maintain accurate and timely clinical documentation.
Assist with patient scheduling and charge posting.
Maintain a safe and organized treatment environment.
Ensure compliance with regulatory, accreditation, and professional standards.
Perform other duties as assigned.
Required Qualifications
Graduate of an accredited Physical Therapy program.
Current and unrestricted South Carolina Physical Therapy License.
Current CPR Certification.
Strong communication, organizational, and documentation skills.
Preferred Qualifications
Previous outpatient rehabilitation experience.
Orthopedic rehabilitation experience.
Musculoskeletal rehabilitation experience.
Manual therapy experience.
EMR documentation experience.
Excellent patient education and communication skills.
