Position Overview
We are seeking a full-time Outpatient Physical Therapist to provide individualized rehabilitation services in Cheraw, South Carolina. This position is ideal for a licensed PT who enjoys helping patients improve mobility, function, independence, and overall quality of life through evidence-based treatment.
The Physical Therapist will evaluate patients, establish individualized plans of care, provide treatment, monitor progress, and collaborate with other members of the healthcare team.
Key Responsibilities- Perform comprehensive physical therapy evaluations and identify patients' functional needs.
- Develop and implement individualized treatment plans based on clinical findings and patient goals.
- Provide therapeutic interventions designed to improve mobility, strength, function, and independence.
- Monitor patient response to treatment and modify plans of care when clinically appropriate.
- Communicate patient progress, concerns, and changes in condition to supervisors and other members of the care team.
- Educate patients, families, caregivers, and nursing personnel regarding exercises and follow-through programs.
- Maintain timely and accurate clinical documentation in accordance with licensing, regulatory, accreditation, and payer requirements.
- Assist with patient scheduling and ensure applicable charges are entered into patient records.
- Maintain therapy equipment and treatment areas in a clean and safe condition.
- Follow applicable professional standards, compliance requirements, and state and federal regulations.
- Complete additional responsibilities as assigned.
Required Qualifications- Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist license in South Carolina.
- Current CPR certification.
- Strong communication and organizational abilities.
Preferred Qualifications- Previous experience in a rehabilitation setting.
- Outpatient rehabilitation experience is preferred.
