Department: Physical Therapy

Schedule Details/Additional Information: Monday, Tuesday, Thursday, and Friday, 7am-5:30pm. Interest or experience in pelvic health is a plus but not required.
Pay Range: $41.10 - $61.65
Job Summary:
As a Physical Therapist at Advocate Health, you will interpret physician referrals, complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations, and establish a plan of care and implement individual treatment based on evaluation results and functional goals. You will also monitor and evaluate outcomes, adjust the individualized treatment plan, and provide status updates to physicians and other teammates.
Responsibilities:
- Interpret physician referrals and complete evaluations
- Establish a plan of care and implement individual treatment
- Monitor and evaluate outcomes and adjust the treatment plan
- Provide status updates to physicians and other teammates
- Identify factors affecting patient comfort and attend to these needs
- Plan for and complete patient discharge, including providing written home programs and referral for community services
- Communicate and collaborate with stakeholders to achieve optimum care
- Complete all required documentation and record patient care and progress
Physical Requirements and Working Conditions:
- Must be able to sit, stand, walk, lift, and squat throughout the workday
- Must be able to lift up to 50 lbs. from floor to waist
- Must be able to lift up to 10 lbs. from waist to overhead
- Must be able to lift and carry up to 40 lbs. at waist height
- Must be able to push/pull with 30 lbs. of force
- Must be able to perform a sliding transfer of 150 lbs. with a second person present
- Must have functional use of senses to allow for effective communication
- Must be able to wear protective clothing as needed
- Must be able to operate all equipment necessary to perform the job

- Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation
- Maintains continuing education requirements as mandated by applicable state professional standards

- Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
